As a risk evaluation, select the worst injury-severity rating which will be caused by failure of a safety-related part of the control system.

F: Frequency and/or exposure to hazard
- F1: Seldom-to-less-often and/or exposure time is short
- F2: Frequent-to-continuous and/or exposure time is long

If it is necessary to perform periodic work on a machine in operation, select F2. If workers rarely have access to the machine, select F1.

If there is little possibility of avoiding hazard when it occurs, choose P2; otherwise, choose P1.
